Weekly 5 Club

FORUM’s Weekly 5 Club Account is like a regular savings account but with a twist.  This account gives you the chance to win prizes or even cash for making regular deposits to your savings.   You will receive a unique set of numbers for each club account you have.  Simply check for winning numbers online, at any FORUM branch location, or call the Weekly 5 Club hotline at 317.558.6095.

FORUM awards one prize per week valued at $10, one prize per month valued at $50, four prizes per quarter valued from $25 to $250, and four prizes per calendar year valued from $100 to $1,500.  Remember, you must “save to play” if you want to “play to win.”

Account Features:

• $25 is required to open the account
• Deposit $5 per prize period to play
• Pick your play period: 3 to 12 months
• $15 fee for early withdrawals
• Must be 18 years of age or older
